Olbing, Essen In den zwanzig Jahren seit dem ersten Bericht iiber Nierenbiopsien bei Kindem 6] hat diese Untersuchungsmethode ihre Bewahrungsprobe be standen und einen festen Platz in der Kindemephrologie gefunden. Einer der wichtigsten Griinde hierfiir ist eine kontinuierliche Verbesserung der Unter suchungstechnik. Strahlensparende rontgenologische, in jiingster Zeit auch sonographische Methoden der Nierenlokalisation gewahrleisten eine hohe Treffsicherheit der perkutanen Nadelbiopsie, welche den Patienten nur we nig belastet. Meist kann die N adelbiopsie in Lokalanasthesie durchgefiihrt werden. Besonders groBe Fortschritte wurden in der Aufarbeitung und Un tersuchungstechnik des Gewebes erzielt. Ultradiinne Schnitte und eine Viel zahl von Farbungen verbesserten die M6glichkeiten einer sehr differenzier ten lichtmikroskopischen Untersuchung. Die Immunfluoreszenzmikrosko pie, die in vielen Fallen Aussagen iiber die Pathogenese bzw. Atiologie er laubt, wurde zu hoher Perfektion weiterentwickelt und kann in den meisten nephropathologischen Zentren durchgefiihrt werden. Das gleiche gilt fiir die Elektronenmikroskopie, die nicht selten eine wertvolle Verfeinerung der Ge webebeurteilung erm6glicht. Hinzu kommt, daB umfangreiche Erfahrungen iiber klinisch-pathoanatomische Korrelationen gerade in den letzten J ahren ver6ffentlicht wurden, die heute bei vielen Patienten eine recht sichere Aus sage dariiber erlauben, mit welcher Wahrscheinlichkeit von einer Nieren biopsie wichtige Aufschliisse zu erwarten sind. Hinzu kommt, daB die Ver standigung zwischen dem Kliniker und dem Pathologen durch die Entwick lung einer ausreichend einheitlichen Terminologie und Klassifikation erheb lich erleichtert wurde.
